Lazily load QR code images when clicked instead of loading them all at once

Update Koha driver to recognize either MD5 or bcrypt password hashes

- Use a profile.d script instead of meddling with /etc/profile directly.

Fetching QR codes for a page of 20 search results can tie up quite a few
Apache workers.  Instead, use JavaScript to load in QR code images on
demand.

Koha 3.14 and later uses bcrypt to hash passwords; previously
it used md5_base64. This patch updates the driver so that it can
handle both.

This partially reverts commit e7e86f7e.

Added needed styling for hierarchy tree to bootstrap.less so that the jsTree theme css doesn't need to be loaded (it breaks hierarchical facet display). Also added a translated "Loading" text with a spinner.
